If you are going to be a part of this team you need to get this document and returned to Coach O'Toole ASAP. it tells you everything you need to know about this team.

This form needs to be signed by your parents and returned to Mr. O'Toole

Physical Form

In order to be a member of the team, every student must have a current physical. Go to your doctor and get a physical. Then have them fill out this form and return it to the school nurse. Finally take the slip of paper the nurse gives you and return it to Mr. O'Toole
